Information Regarding Chapter 13

Effective December 1, 2017, amendments to several federal and local rules and forms will become effective. Primary among them will be the change in Chapter 13 plan forms. The U.S. Bankruptcy Court for the District of Massachusetts did not adopt the national plan (Official Form 113) and instead adopted its own local Chapter 13 plan form (Official Local Form 3). This page is designed to provide you with helpful information regarding the new forms and rules which will become effective on December 1, 2017.

Exhibits 1-4 of the Chapter 13 Plan (effective December 1, 2017) are located below. These duplicate forms are found in the Plan but may be used if you are filling out the exhibits separately or need additional pages for those forms.
