On May 19, 2025, the Court issued an Order Adopting Appendix 1, Chapter 13 Rules, and Certain Local Forms which can be found here.  In it, the Court adopted Appendix 1 which reflects the prior Standing Order revisions to Appendix 1 between August 1, 2016, and May 1, 2025.  Appendix 1 can be found here.  By the Order, the Court also adopted Official Local Forms 3, 3A, 3B, 4, 8, 17, 19, 21, and 22 which can be found here.
